Developing Measures of Agency for Social and Behavioral Change (SBC) programs
Agency is a fundamental concept in Social and Behavior Change (SBC) programming, as it lies at the heart of decision-making and empowerment processes. However, significant gaps remain in how agency is conceptualized, measured, and understood, particularly in terms of its impact on health and well-being. These gaps hinder our ability to compare agency across different contexts and to draw lessons that can enhance empowerment-focused SBC interventions. Current measures of agency often oversimplify the concept by focusing solely on decision-making, without recognizing its broader and more complex dimensions. Furthermore, these measures are typically developed without direct input from the individuals they aim to support, making them less relevant or effective in capturing the lived experiences of the target populations.
To address these gaps, Agency for All consortium partners – EVIHDAF in Cameroon, Makerere University in Uganda, and Sambodhi Research & Communications Pvt. Ltd. in India—are collaborating to develop culturally relevant, reliable, and valid measures of agency. This initiative, running from 2023 to 2025, aims to fill the global gap in high-quality, cross-cultural agency measures while strengthening the capacity of local partners to engage in the development and application of these tools.
Over the first year of Agency for All activities, several actions have been undertaken to lay the groundwork for this study, deepening our understanding of agency and its measurement across different contexts. These activities have also helped define the priorities and shape the concept note for the study, ensuring that the research is grounded in local realities and addresses the specific needs of the populations involved.
Study objective
The overall goal of this study is to understand how women and men in three different Agency for All focal geographies understand agency within their own lives and communities, and to use that knowledge to develop measures of agency. The study objective, therefore, is to develop cross-nationally validated measures of agency informed by local conceptualizations of agency across health outcomes and diverse contexts.
Study Approach
In implementing this study on Agency measures for SBC programs, the following phases will be
- Phase 1 – Exploring the meaning of agency: we will conduct iterative stages of qualitative research to inform item pool and scale development.
- Phase 2 – Item pool and scale development: we develop the two main components of the survey to be enumerated in Phase 3: the agency scale and the cultural consensus module statements.
- Phase 3 – Scale evaluation and validation: we will evaluate and validate the agency scale using best practices in scale development.

Targets: 1) Explore, discuss, and document practitioners’ perspectives on how to conceptualize and measure agency, as well as identify priority questions relevant to social and behavior change (SBC) programming across sectors. 2) Assess how experts working in SBC research and programming conceptualize and measure agency, and understand how it differs across sectors and geographies. 3) Explore how agency is currently measured and conceptualized in low- and middle-income contexts across various health domains. 4) Conduct research on the development and evaluation of a culturally relevant measure of agency for global health, using innovative ethnographic and psychometric methods tailored to different contexts.
